You can’t just continue what you were doing while it rings, you have to freeze and hardly breathe while you stare at it like a deer in headlights so they don’t hear you through the unanswered phone or God forbid you move wrong and it accidentally picks up.
I sit there wondering, if I hit the red button to reject the call, will the caller know? Or will the phone continue to ring on their end? Don’t want them to know I deliberately ignored their call…

That’s not ignoring, that’s rejecting. Turning down vs. ghosting if you will.
For me it’s merely because I cannot concentrate with the noise of whatever ringtone is playing (any ringtone).
